UB04 Admission Source Code 7: ER

Introduction

UB04 Code 7 is the specific indicator for Emergency Room admissions. This code is used in Field 15: Admission Source to identify patients who entered the facility through the emergency department.

Usage

When billing with UB04 Code 7, the provider is indicating that the patient's admission to the hospital was initiated via the facility's own Emergency Room. This code is critical for accurate reimbursement as it distinguishes between scheduled admissions and urgent, unplanned care. To use this code correctly, the patient must have been evaluated and treated in the ER prior to the formal inpatient admission order. It is frequently paired with Revenue Code 0450 to ensure consistency across the claim form.

While Code 7 is specific to ER admissions, other codes within Field 15 exist to define different points of origin, such as Code 1 (Non-Healthcare Facility Referral), Code 2 (Clinic Referral), or Code 4 (Transfer from a Hospital).
