UB04 Admission Source Code C: HHA Readmission

Introduction

UB04 Code C, specifically designated for HHA Readmission, is a critical data element used to track patient movement within the healthcare continuum. This code is used in Field 15: Admission Source. It identifies patients who are returning to home health care services after a previous discharge or transition.

Usage

When billing with UB04 Code C, the provider is indicating that the patient is being readmitted to a Home Health Agency (HHA). This code should be utilized when the patient was previously under the care of the same or a different HHA and is now initiating a new episode of care. Accurate reporting of Code C is essential for Medicare and private payer reimbursement, as it helps determine the start of a new 30-day period or 60-day episode and ensures that the claim is processed under the correct home health prospective payment system (HHPPS) guidelines.

While Code C is specific to HHA Readmission, other codes exist in the parent field to represent different points of entry, such as Code 1 (Non-Healthcare Facility Point of Origin), Code 2 (Clinic), or Code 4 (Transfer from a Hospital).
