About FormJuicer

FormJuicer is a form data extraction platform that converts PDF forms into structured, usable data using advanced OCR. From insurance claim forms to tax documents, financial statements to healthcare records — if it's a form, FormJuicer can extract it. UB-04 and CMS-1450 are currently supported, with more form types expanding based on demand. We built FormJuicer because manual data entry from forms is slow, error-prone, and expensive across every industry. And we built this knowledge base because understanding what's on those forms is just as important as extracting it.

Our Product

FormJuicer's core product is an OCR pipeline that handles any structured form:

  • Upload PDF forms — scanned or digital — across any supported form type (UB-04, CMS-1450, and more based on demand)
  • Extract specific field data by form type — revenue codes and patient demographics from claim forms, line items from invoices, account numbers from financial forms
  • Export to Excel (.xlsx), CSV, or structured JSON for integration with your existing systems

We're actively expanding to support additional form types based on user demand — from CMS-1500 variations and EOBs to W-2s, 1099s, and beyond. It's built for batch processing, handles low-quality scans, and maintains compliant data handling throughout.

Why a Knowledge Base?

Extracting data from a form only helps if you understand what each field means. Our knowledge base currently covers medical billing forms because that's where our users are today — but as we expand FormJuicer to new form types, our reference content will expand too. We created this reference because:

  1. Our users ask us these questions daily — "what does revenue code 0450 mean?" or "why was field locator 42 denied?"
  2. We know these forms intimately — building OCR for structured forms requires understanding every field, every code set, and every rule that governs them
